Welcome to NAK TAE Belfast.

Your safety and wellbeing are our priority. Before participating in Muay Thai training, all members aged 18 and over must complete our Adult Membership, Participation & Waiver Form.

This includes your personal information, emergency contact details, health declaration, club code of conduct, acknowledgement of training risks and optional sparring consent.

Please read each section carefully and provide accurate information before signing and submitting your form.

**HEALTH & FITNESS DECLARATION** Muay Thai is a physically demanding combat sport involving exercise, physical contact and, where applicable, controlled striking. Please tell NAK TAE about any medical condition, injury, medication, allergy or other circumstance that could reasonably affect your ability to participate safely.
I confirm that the information I have provided is accurate and complete to the best of my knowledge. I agree to inform NAK TAE and/or its coaches if my health, fitness or medical circumstances change in a way that may affect my ability to participate safely.
**NAK TAE CODE OF CONDUCT** NAK TAE is committed to providing a safe, respectful and positive training environment. **All members are expected to:** - Treat coaches, staff, training partners and other members with respect. - Follow reasonable instructions given by coaches and staff. - Train responsibly and within their own capabilities. - Use appropriate protective equipment when required. - Maintain good personal hygiene and appropriate training attire. - Report injuries, illness or concerns to a coach. - Respect the boundaries and wellbeing of training partners. - Use controlled and appropriate contact during partner drills and sparring. - Stop immediately when instructed to do so. - Look after NAK TAE equipment and facilities. - Behave responsibly inside and outside the training environment when representing NAK TAE. **The following behaviour is not acceptable:** - Bullying, harassment or intimidation. - Discrimination or abusive behaviour. - Deliberately dangerous or reckless conduct. - Deliberately attempting to injure another member. - Aggressive or threatening behaviour. - Unwanted physical contact. - Training or sparring while impaired by alcohol or recreational drugs. - Ignoring reasonable instructions from coaches. - Repeatedly disregarding club safety rules. NAK TAE reserves the right to refuse or withdraw participation where a person's behaviour presents a safety or welfare concern.
**MUAY THAI PARTICIPATION & RISK DISCLAIMER** I understand that Muay Thai is a physically demanding combat sport. Participation may involve strenuous exercise, physical contact and activities including, but not limited to: - Pad work - Heavy-bag training - Footwork and movement drills - Conditioning exercises - Partner drills - Clinching - Controlled technical contact - Sparring I understand that participation in these activities carries inherent risks. **Possible injuries may include, but are not limited to:** - Bruising - Cuts and abrasions - Muscle strains - Sprains - Joint injuries - Dislocations - Fractures - Dental injuries - Head injuries - Concussion - Other physical injury or illness I understand that safety measures, coaching, protective equipment and appropriate supervision can reduce risks but cannot eliminate them completely. I agree to participate within my own physical capabilities and to tell a coach if I feel unwell, injured, unsafe or unable to continue. I understand that I am responsible for informing NAK TAE of relevant medical conditions, injuries or circumstances that could affect my safe participation.
**LIABILITY & WAIVER** I voluntarily choose to participate in activities provided by NAK TAE. I understand that participation in Muay Thai involves inherent risks and that injuries may occur despite appropriate coaching, supervision, rules and safety precautions. I agree to follow NAK TAE's rules, safety procedures and reasonable instructions from its coaches and staff. I acknowledge that I should not participate in training or sparring if I am unwell, injured, impaired or otherwise unable to participate safely. Nothing in this agreement is intended to exclude or restrict any liability, responsibility or legal right that cannot lawfully be excluded or restricted. To the extent permitted by applicable law, I accept responsibility for the ordinary and inherent risks associated with my voluntary participation in Muay Thai activities at NAK TAE. I understand that NAK TAE may stop or restrict my participation where a coach or staff member reasonably considers that continuing would create a safety risk to me or another person.
**SPARRING DECLARATION & CONSENT** Sparring is a separate activity involving controlled physical contact and carries additional risks. Before sparring at NAK TAE, I understand that: - Sparring involves intentional physical contact. - I may receive strikes to permitted areas of the body. - Even controlled sparring can result in injury. - Protective equipment does not eliminate the risk of injury. - I must follow the instructions of the coach supervising the session. - I must maintain appropriate control and intensity. - I must respect my training partner and their limits. - I must immediately stop if instructed by the coach. - I must immediately report any injury, suspected concussion or other concerning symptoms. - I must not spar if I am injured, unwell or impaired. - I must inform the coach of any relevant medical circumstances before sparring. I understand that I can decline to spar and that I should communicate any concerns to the coach before or during a sparring session.
**COACHING & SAFETY** I agree to: - Follow instructions given by NAK TAE coaches. - Use equipment appropriately. - Wear any protective equipment required for the activity. - Inform a coach immediately if I am injured or feel unwell. - Stop an activity when instructed. - Respect training partners and other members. - Not deliberately engage in behaviour that could cause unnecessary injury. I understand that a coach may modify, stop or restrict an activity where they consider it necessary for safety.
**PHOTOGRAPHY & VIDEO** NAK TAE may occasionally take photographs or video footage during training sessions, events or club activities. Please indicate your preference. Your choice will not affect your ability to participate in training.
**PERSONAL INFORMATION** NAK TAE will collect and use the information provided in this form for purposes including: - Membership administration - Contacting members - Emergency contact - Managing participation and safety - Maintaining appropriate club records - Communicating club information NAK TAE will handle personal information in accordance with applicable data protection requirements and its Privacy Notice. I confirm that the information I have provided is accurate and understand that I should notify NAK TAE if relevant information changes.
**FINAL DECLARATION** By submitting this form, I confirm that: - I am 18 years of age or older. - I have provided accurate and complete information. - I have read and understood the NAK TAE Code of Conduct. - I understand that Muay Thai is a physical combat sport with inherent risks. - I understand that injuries can occur even when appropriate safety measures are followed. - I agree to follow the instructions of NAK TAE coaches and staff. - I agree to inform NAK TAE of relevant injuries, medical conditions or changes in circumstances that may affect my safe participation. - I understand the additional risks associated with sparring. - I voluntarily agree to participate in NAK TAE activities subject to the terms of this agreement. - I understand that this agreement does not seek to exclude or restrict any liability or legal rights that cannot lawfully be excluded or restricted.
